Job Description - Support Coordinator

Want to be part of this? We’re currently recruiting for a Support Coordinator to join our Housing Services team.

 

What you’ll achieve in the role: 

 

The overall purpose of this role is to ensure efficient and effective management of designated sheltered schemes whilst promoting independence and facilitating support for every resident.

 

Key responsibilities include:

 

  • Deliver support to customers living in housing schemes for older people or as part of associated floating support services to vulnerable people  in line with their individual needs.
  • Assess all new residents moving into sheltered schemes to determine which of the three support levels would most appropriately meet the customer’s individual level of need.
  • Undertake regular contact with customers, including visits, telephone calls, emails, and text messages in order to provide support and enable them to live independently whilst promoting dignity, choice and confidentiality.
  • Proactively promote and encourage independence within customers’ own homes, including the use of assistive technology.

 

What you’ll bring to Raven: 

 

For this role, we are looking for someone with the following:

 

  • A good understanding of the housing support needs of older people.
  • A full, clean driving licence with access to own car.
  • Excellent communication skills, both written and verbally, to a wide variety of different audiences.
  • Strong and demonstrable IT skills, including using bespoke systems and Office 365.

DBS clearance will be required for this role.
